Safety, honesty, respect and the proper route

There is no universal slogan that answers every scenario. The useful questions are concrete: Who could be harmed? What information is protected? Is the person acting within their role? Has the concern been raised clearly and proportionately? Has the immediate problem been corrected?

SJT principles

A compact decision sequence

  1. Check immediate safety. Address a credible risk promptly.
  2. Stay within competence and authority. Ask for supervision or use the correct route.
  3. Protect confidentiality and consent. Names are not the only identifying information.
  4. Be honest about errors. Correct the problem and inform the appropriate person.
  5. Speak clearly and proportionately. Specific private challenge is often stronger than accusation or silence.
  6. Respect patients and colleagues. Support people without concealing risk or misconduct.

Each response should be judged independently. More than one action in a scenario can receive the same rating.
